Division of “HIV Infection and Infectious Control”

“HIV and AIDS represent global disaster, are one of the most considerable calls to development, progress and stability of our societies and the world in general and demand acceptance of emergency and complex measures of global reaction and the account of the fact that spread of HIV quite often is a consequence and the reason of poverty” (the 65th session of the United Nations General Assembly, 2011).

“… more than one third of all new cases of infection of HIV falls to the share of young people in age group from 15 to 24 years, and about 3000 young people daily become HIV-positive people”, “… … the number of new cases of infection of HIV grows in Eastern Europe and Central Asia” (the 65th session of the United Nations General Assembly, 2011).

In Kazakhstan, according to the National AIDS Center, the estimated number of HIV infected is over 24 000, the number of the HIV-infected women is increasing. If at the beginning of epidemic the potential mode of transmission prevailed due to injecting drug use. Since 2011 the heterosexual mode of transmission which is hard to control, started to dominate. Besides, HIV can be transmitted as healthcare associated infection.

For the response to this serious crisis of public health care, medicine in general and purposeful training concerning counteraction of HIV infection in Kazakhstan in January, 2011 in KAZNMU he S. D. Asfendiyarova has organized the course “HIV Infection and Infectious Control” transformed in June, 2012 to the module of the same name as independent structural division. Since March, 2011 also the course at Institute of post degree education functions. Except the questions connected with HIV infection, training is also conducted on a problem of infectious control of the infections connected with delivery of health care.

Basis of educational process at the “HIV Infection and Infectious Control” module is UNESCO concept about training, as about “process which connects experience of the person (at the level of the certain individual and from an external environment) and various influences for acquisition, enrichments or changes of knowledge, skills, values, the relations, behavior and views of the world”. Need of transformation of the purposes and daily practice of training according to new requirements of the 21st eyelid about need of knowledge of the main subjects and specific subjects of 21 eyelids, possession of technologies, innovative and social skills is thus taken into account.

Fluency in English allows the module lead to update constantly a training material according to the last achievements of world science and practice on HIV infection and infection control. When carrying out occupations on the module the innovative pedagogical approaches recommended for the best assimilation, storing and application of a training material are used.

Besides traditional lectures, various innovative forms of education are used: clinical analyses, case studies, role games, discussions in small groups, “brain storming”, seminars, thematic conferences, PowerPoint presentations. According to UNESCO, the most effective method of training in the 21 century is group projects to gain not only knowledge but also social and communication skills. Introduction of this method is planned. Students who are trained on the module of 12 days observe and analyze observance of standard precautionary measures by medical personnel of clinical bases of the module and establishments where they can work.

The Division is located at the Almaty City AIDS Center which provides care and treatment for all PLWH who reside in the City and its suburbs. Also PPS of the module is based in the Regional center for prevention and fight about AIDS of Almaty region, City student’s policlinic of Almaty.

Nugmanova Zh. S., MD, PhD, Prof. under her management two doctor’s and 12 master’s theses are defended, has over 200 published works, including three monographs in a co-authorship, articles in the foreign reviewed magazines with импакт a factor

She has long-term experience in the international organization, is fluent in English, gives lectures, gives classes in English, repeatedly trained abroad concerning management of health care, epidemiology, to questions HIV, infectious control, distance learning.

Prof. Nugmanov Zh.S. I was a consultant of UNICEF, the tutor of the Regional distance training program on HIV and AIDS for health workers of Central Asia, the EUNIDA/GIZ project concerning HIV and infectious control, the consultant of UNODC. Nugmanova Zh.S. is a member of the International society AIDS (International AIDS Society), Honorary professor of TashMI 2, on a voluntary basis I was a cofounder and the member of council Popechitelya of Public fund of the “Boat” created with assistance of the World bank according to the Agreement between the governments of Kazakhstan, the United States of America and the Swiss Confederation for support of children and youth from needy families in Kazakhstan.

Results of her work with the international organizations were:

  • the conclusion of the contract on cooperation between KAZNMU of S.D.Asfendiyarova and School of public health care of University of the State of New York in Albany, the USA;
  • together with representatives of the USA employees of the module carry out scientific researches on a HIV problem within an intra high school competition of grants of KAZNMU;
  • the Memorandum of cooperation in distance learning between KAZNMU, Institute of retraining of medical shots, Kyrgyzstan, the Republican Center of family medicine, Tajikistan and the Tashkent institute of improvement of doctors is concluded under the auspices of GIZ;
  • between the EUNIDA/GIZ project and KAZNMU concerning infectious control and work with vulnerable groups of the population. Within the last project innovative DVD on infectious control is developed.
  • The agreement with ICAP/CDC as a result of which in 2014 the methodical grant “HIV – an infection” is published, 234 p.

In labor activity has practical experience of out-patient work in rural conditions; I worked in a number of city hospitals, including cardiologic, rheumatologic and geriatric offices. By results of work with difficult category of patients with the status of refugees in RK protected dissertation work. Now closely takes up the problems of HIV infection.

Assistant Kurmangaliyeva G. S. – the main direction therapy, in labor activity has practical experience as the doctor as the therapist in regional hospital and after consultation of pregnant women in female consultation. I finished residency in “infectious diseases” on chair of infectious and tropical diseases with the course “HIV Infection and Infectious Control” of KAZNMU. Now deals with HIV infection issues, advises patients with HIV an infection in city clinics.

Elena M. Kym, the assistant to the module, the infectiologist of the highest category with length of service more than 40 years. Has invaluable practical experience of work both with out-patient, and with inpatients with any infectious pathology. One of the first doctors who introducing in practice treatment and supervision of patients with HIV infection in Kazakhstan.

Educational and methodical work

According to Working Educational Plan in 2013-2014 academic year the 3-d course students of specialties 051102 “Public health”(SOSE- 2006), 5В110400 “Medico-prophylactic business”( SOSE – 2009), the 4-th course students of specialty 051301 “General medicine” study at the department. The 5-th course students of specialty “General medicine” study elective discipline “hospital management”.

According to SOSE- 2006 the 6-th course interns of specialty “General Medicine” are trained on direction “Internal Medicine”,051302 ” Stomatology”.

In all disciplines the educational methodic complexes of discipline (EMCd) are developed in languages of studying, the competence-based approach is introduced with the development of methods for assessing 5-competencies (knowledge, practical, communication, legal skills and the skills of self-improvement) for each course and faculty with regard to a future specialization.

Clinical operation of the “HIV Infection and Infectious Control” module

The “HIV Infection and Infectious Control” module is clinical base the Center for prevention and fight about AIDS in Almaty. The center is organized on June 4, 1990, this public medical institution which is engaged in preventive, anti-epidemic actions for the prevention of distribution of HIV infection in the territory of the city and appearing medical care in revealed HIV-infected and the patient AIDS.

“HIV Infection and Infectious Control” module renders assistance of the Center according to the national program of fight about HIV/AIDS are:

– preventive programs among teenagers, youth and the persons practising risk behavior;

– the prevention of transfer of HIV from mother to the child;

– identification, dispensary supervision over HIV – infected and patients AIDS and their treatment;

– education of the population and creation in the society of the tolerant and sympathetic relation to HIV – infected and the patient AIDS, as well as to other patients.

Epidemic HIV/AIDS of Almaty is at the concentrated stage. The program of prevention of transfer of HIV from mother to the child captured 100% of the been born children. On the basis of medical establishment openly 16 points of “Trust”, 7 friendly clinics and 40 offices of psychosocial consultation.

The center actively cooperates with the International and non-governmental organizations, such as UN/AIDS, Global fund on fight against malaria, tuberculosis and AIDS, USAID, UNAIDS, “PSI” which is “The Equal — Equally”, “Trust Plus”, Association “ANTISPID”, “Adali”, “Kazakhstansky soyuz lyudey, zhivushchikh s VICH”, etc.

In the Center departments treatment-and-prophylactic, laboratory, prevention, an epidemiological surveillance behind HIV cases work. The AIDS center of the city of Almaty is considered one of the best in the Republic where highly qualified specialists among whom 2 candidates of medical sciences, 71% of doctors and average medical workers have the highest qualification category work. The center is equipped with modern kliniko-laboratory equipment thanks to what patients with HIV infection can receive the highly skilled help according to the last WHO recommendations.

Employees of the module take active part in improvements of quality of the provided medical care to patients with HIV infection, to adults and children. One of actual problems for practical doctors of the AIDS Centers – disclosure of the diagnosis to teenagers with HIV infection. On September 14-16, 2015 in the city of Tashkent with assistance of UNICEF there has taken place the sub regional seminar on the psychological and clinical assistance for the teenagers living with HIV. The seminar was held international the trainer from Great Britain by Magda Conway and Amanda Elie. The assistant to the module Akhmetova G. M., having had training at this seminar, advises doctors, parents and teenagers concerning disclosure of the HIV status to children and teenagers.

Safe donorship

With assistance of the faculty of KANZNMU of S. Zh. Asfendiyarov – MD the deputy professor of Zh. Z. Trumova, experts of GU “The Almaty Regional Center for Prevention and Fight about AIDS” on September 16-17, 2014 in Almaty, Kazzhol hotel (a conference hall “Baikonur”) and participation of the Republican center of blood (director D. S. Bekirov) and, the Almaty state institute on improvement of doctors (associate professor V. V. Chursin) have held the 2-day training seminar for deputy chief physicians and doctors of transfuziolog of the medical organizations of area. 74 experts have participated in a seminar: from 36 MO areas (66 doctors) and the AIDS center (8 doctors). At the end to participants certificates have been issued.

“Only having combined efforts of all health workers, it is really possible to reach continuous improvement of quality and safety of components of blood and the method of a transfusion, reducing risk level for personnel, donors and patients, increasing reliability of donor material” — such thought wanted to bring to the experts participating in a seminar, organizers.

Safety on a workplace and the tolerant relation to PLHIV.

With health workers of the city of Almaty seminars, trainings, round tables for the purpose of improvement of quality and safety of medical care are provided. The health workers and people involved in HIV epidemic from a position of mutual negative attitude to each other turn into partner mutually beneficial cooperation. Prevention of transfer of HIV from mother to the child.

In 2015 by order of OUZ of Almaty region (head A. Bayduvaliyev) with assistance of the faculty of KAZNMU of S.D.Asfendiyarova (the rector MD professor Akanov A.A), the trainer of the teacher of MD Trumova Zh.Z,

3 training seminars trainings and master class for experts are held from May 06 to May 30, 2015 to the period (doctors obstetricians-gynecologists, midwifes of area concerning PPMR to Almaty and Taldykorgan.

Representatives of the international organizations participated in a seminar: % the head of YuNEYDS is Mr. Goliusov A.T., director of the Qualitative Health Care project Kudusova E.M. and other invited persons.

In total over 100 experts are trained, to participants certificates have been issued.

International cooperation

Module PPS actively cooperate with colleagues from foreign countries and the international organizations. So, since 2008, the module lead is the teacher of a Central Asian course of the distance learning (DL) of a problem of the HIV conducted on a regular basis for doctors of Kazakhstan, Kyrgyzstan, Tajikistan and Uzbekistan with GIZ support, Germany. In August – December 2011 it passed training in UAlbany, SUNY, US, in the area of SUNY to the International training and research program on AIDS and to the connected infections, Fogarti financed by the International center at National institutes of health of the USA.

At the initiative of PPS of the module were concluded:

  • The memorandum of cooperation on development of distance learning between KAZNMU, Institute of retraining of medical shots, Kyrgyzstan, the Republican Center of family medicine, Tajikistan, the Tashkent institute of improvement of doctors under the auspices of GIZ;
  • Contract on cooperation between KAZNMU and School of public health care of University Albany, SUNY, USA;
  • The memorandum of cooperation between KAZNMU and the Project of the European Union/GIZ concerning HIV and koinfektion and infectious control

In April, 2015 KAZNMU together with the New York state international program of training and researches, the USA, the Center of Studying of Global Health in Central Asia, the AIDS Republican Center has held the International scientific and practical conference on researches in the field of HIV “Realization of scientific approaches to treatment and care of HIV infection in Kazakhstan”. At conference Malaziya’s data have been submitted, to Estonia, Georgia, Ukraine, and also Kazakhstan on the conducted researches in the field of HIV infection.

Educational operation of the “HIV Infection and Infectious Control” module

By day of memory of the dead from AIDS the large-scale action among student’s youth of the city of Almaty “In solidarity” was held.

Any wishing could pass express testing for HIV, participate in a quiz “A safety route”, read about destinies of the real people living with HIV (PLHIV). After speeches of teachers, LZhV and students, there took place discussion on the subject “Stigma and Discrimination in Society in relation to HIV-positive People”.

The action huge квилта with names of the children who have lost from AIDS and adults came to the end with carrying out. In solidarity to people the victim from AIDS all present rose and included illumination of mobile phones. This action is the important educational action forming at youth the tolerant relation to any person, isn’t dependent on religion, a nationality, the HIV status.
